It is amazing how the human body can often adapt and compensate, even to the greatest insults.

This is the LV of a patient with cardiomyopathy that underwent an emergency VT ablation under GA, only requiring minimal vasopressor support throughout.

Highlighting and writing in the margins of my vasopressor and inotrope handbook and my coworkers are making fun of me for it but they won't be laughing when I can tell them the the half life of every medication their patient is on!

I think it's unnecessarily caustic (and largely incorrect) to call it virtue signalling. Everyone has different risk thresholds, and their own mitigation strategies. I'm not generally a drawer-upper but I see no reason to demean those who know they'd be stressed getting a vasopressor in a hurry.

I've seen it used as a last ditch vasopressor like four times in ten years but a lot of folks seem to think taking it has some benefit. I think dyeing your teeth blue to live longer is a dubious proposition.

Hill 9. (more of a hillock) If you have time, site an arterial line before induction – failing that have an assistant keep a finger on the radial pulse and give more vasopressor if it weakens (this is very much second best).

The value judgments in the #ADAPT trial design are more interesting than the results

I give broad spectrum antibiotics to almost every patient on a vasopressor in the MICU. It’s because I value human life orders of magnitude more than antibiotic “stewardship” in my decision making
This is just ludicrous misvaluation of the relative importance of one day of antibiotics vs a human life
