What is the most efficacious introducer when performing hyperangulated videolaryngoscopy?

Stylets were most efficacious and a static bougie least.

hyperangulated videolaryngoscopy pearl:

this is your daily reminder to *look* at the oropharynx while you’re passing the ETT through it.

don’t stare anxiously at the screen while blindly jabbing at the oropharynx, waiting for the tube to magically appear on the screen. #EMIMCC
During hyperangulated #videolaryngoscopy #HAVL the ETT was passed through the soft palate

The authors discuss a ‘blind spot’ of the oropharynx during videolaryngoscopy.

Any thoughts?

Clinicians with previous experience in direct laryngoscopy required an average of 12 tracheal intubation attempts with hyperangulated videolaryngoscopy to achieve an acceptable first-attempt tracheal intubation success rate.

If a clinician is already experienced in direct laryngoscopy, how long does it take to achieve proficiency in hyperangulated-blade videolaryngoscopy?

Approximately 12 attempts!

agree 👇

with roc & videolaryngoscopy it’s generally easy to reinsert the laryngoscope *without* removing the ETT

if you & everyone in the room can plainly see the ETT going thru the cords, its placed correctly

this is *different* from one operator glimpsing the cords for a split second #EMIMCC
“No trace = wrong place” should be retired & replaced by the requirement to exclude oesophageal intubation if ‘sustained exhaled CO2’ is not detected.

Should we use universal videolaryngoscopy in real-world operating theatre settings?

YES!

↑ rate of easy intubation
↑ rate successful first-attempt intubation
↓ incidence of difficult laryngoscopy
↓ complications related to intubation

In this study, videolaryngoscopy was 74-fold more likely to improve the direct laryngoscopy view by at least 1 grade than vice versa.

DL is highly unlikely to improve the laryngoscopic view if it is difficult with a Mac VL.

Should you use videolaryngoscopy to confirm tracheal tube location?

Further use of VL to evaluate location could lead to ongoing misinterpretation.

In patients living with class 2 or 3 obesity (BMI ≥ 35 kg.m-2), videolaryngoscopy significantly reduced:

• failed tracheal intubation incidence
• first-attempt failure incidence
• incidence of hypoxaemia
• poor glottic visualisation

The ease of tracheal intubation with videolaryngoscopy cannot be inferred from glottic view alone.

A structured approach that incorporates what was used, what was seen and what was done may improve communication in clinical practice.
