In 973 BioFINDER-2 participants with fMRI + CSF/PET biomarkers, we modelled age- and AD-related connectivity changes across the cortex. Two distinct patterns emerged: AD pathology → sensory-association axis (r=.74); age → representational-executive axis (r=.75)

Different functional connectivity gradients reflect aging and Alzheimer's disease
Aging and Alzheimer's disease (AD) are accompanied by alterations to large-scale communication patterns in the brain, which can be tracked in vivo using functional connectivity. The location, direction and relevance of these changes remain widely debated, although they are rarely studied in the context of whole-cortex communication dynamics. Here, in two independent cohorts (BioFINDER-2, N = 973; Alzheimer's Disease Neuroimaging Initiative, N = 129), we show that functional connectivity changes...

Different functional connectivity gradients reflect aging and Alzheimer's disease
We analyzed resting-state functional magnetic resonance imaging (fMRI) data from the BioFINDER-2 cohort (973 participants) with complete data (cerebrospinal fluid (CSF) Aβ42/40, tau PET and fMRI scans; see Table 1 for participant characteristics). After parcellating the cortex into 1,000 regions33, FC was quantified using nodal affinity, the parcel-wise average of connectivity similarity to all other parcels. Henceforth, we focus on this FCS as our central FC measure of interest (Fig. 1a and Methods). Nodal affinity was used as the outcome in parcel-wise regression models (Methods), with the t-values of the terms of interest mapped onto the cortical surface, resulting in ‘t-maps’ (Fig. 1a,c). Our goal was to assess the degree to which these t-maps aligned with the principal axes (that is, gradients) of functional organization (Fig. 1c). The gradients of interest were defined as the first three principal components of the averaged connectomes from healthy controls (Fig. 1b; see Extended Data Fig. 10 for comparisons of different methods to derive gradients, and explained variance of the derived components). Supplementary analyses (Supplementary Table 1) with gradients derived using several methods (including the traditional diffusion map embedding from Margulies and colleagues20) confirmed that this methodological choice did not substantially impact the results...

Using data from the BioFINDER-2 cohort and an independent validation cohort (Knight ADRC), we investigated whether plasma biomarkers could approximate the recently proposed amyloid- and tau-PET–based Alzheimer’s disease staging framework in a scalable and minimally invasive way.

New BioFINDER preprint!
We formalise the Aβ–tau–neurodegeneration (ATN) framework into a mechanism-based model of Alzheimer’s disease, enabling us to simulate longitudinal imaging biomarkers and study how disease processes evolve and interact across the AD continuum.
