Once again, for those at the back, the effort to refute bulls*t is 10x that required to produce it. Can we please stop doing high dose Vit C trials in critical care now?

It was wonderful to have you join us @ccctg.bsky.social at #CCR26!

4 superb Canadian, or Canadian led, trials no less!

#FISSH #SAVE-ICU #VICTORY & #Oseltamivir (with REMAP-CAP)

Looking forward to more great trials from the original critical care trials group

#CCR26 ABC post-ICU - RESULTS
Liberal vs. restrictive transfusion strategy post-ICU: more blood used in liberal group (~2 units)

Primary outcome: No difference in SF-36 PCS at 90 days
No difference in sensitivity analyses

Does this re-assure re. allowing low Hb post-ICU?
June 12, 2026 at 2:49 PM
#CCR26 Last trial of the day, ABC post-ICU evaluating liberal RBC transfusion (aiming Hb 100-120) vs. restrictive (Hb 70-90).

#CCR26 GASTRIC-PICU study editorial is provided by Dr Srinivas Murthy (Vancouver), and begins by also commenting on the brilliant trial logo for the study, and surveying the in-person audience about their routine practice re. GRV!

What do you do (adults and paediatrics) re. GRV?
June 12, 2026 at 10:34 AM
#CCR26 GASTRIC-PICU results were presented by Prof Lyvonne Tume and ProfDavid Harrison

Results are now available in @JAMA_current and the panel discussion will include Dr Chris Seymour (Assoc. editor for critical care at JAMA)

#CCR26 GASTRIC-PICU:
Survival and days free of IMV at 30d: 25 (21-27) days in both groups (adjusted OR, 0.95 [95% CI, 0.86-1.05]; within non-inferiority margin.

Mean % est. energy requirements by 72 hours: 80.3% intervention vs. 76.8% usual care; P < .001

Practice changing?

#CCR26 GASTRIC-PICU:
P: Children ≤16y fed via gastric route + receiving IMV
I: No routine GRV measurement
C: Usual care (inc. GRV assessments ≤6h)
O: Co-primary: Composite of survival MV-free days (noninferiority) + % estimated energy requirements achieved by 72h (superiority)
June 12, 2026 at 10:17 AM
#CCR26 Gastric-PICU aimed to determine the clinical and cost-effectiveness of not routinely measuring gastric residual volume (GRV) to guide enteral feeding and to determine if it is inferior to standard, at least 6h, GRV assessment in mechanically ventilated children in PICU

#CCR26 Dr Serpa Neto reminds us of the physiology underpinning sodium bicarbonate, before talking through some past studies of sodium bicarbonate in metabolic acidosis. Interesting to see the variation in practice with sodium bicarbonate!

Do you use bicarb for metabolic acidosis?

#CCR26 Friday, SODa-BIC to get us started. Our second bicarb study of the week! Chaired by Prof Charuhas Thakar (Queen's University Belfast) and the trial is presented this morning by Dr Ary Serpa Neto

#CCR26 ACCOMPLISH trial - no difference in number of days spent at home within 90 days after hospital discharge between usual care and the four remote monitoring arms

But Dr Yende shows some data that suggests there may be heterogeneity of treatment effect based on patient age
June 11, 2026 at 2:51 PM
#CCR26 ACCOMPLISH was an adaptive platform trial that aimed to compare effectiveness of different care models to prevent readmissions for patients hospitalized with sepsis or LRTI. The primary outcome was number of days spent at home within 90 days after hospital discharge.
