6) To digg deeper into PEM manifestations, I think we need better questions than those from the DePaul symptom questionnaire - these focus too much on fatigue after exertion and fatiguability, not on becoming sicker much later after the (over)exertion.

We have little trust in these NIH researchers because of their problematic effort preference theory.

This experimental setup, however, might help to show if muscle fatiguability in ME/CFS has a central (brain) or a peripheral (muscle) origin or both.

Got diagnosed with occult #TetheredCordSyndrome today after a loooong while of progression and am urged to have surgery for it next year >"< it’s said that my leg fatiguability is likely too far gone to be really fixed with surgery but the other symptoms should be reduced yayay!

6) The changes in fMRI (signs of central fatiguability) also occurred, but only later. The authors therefore conclude: "While we found evidence for fatigability of central and peripheral 60 origins, peripheral fatigue seems to occur first."

7) Fatiguability will be measured following tasks that induces cognitive fatigue (PASAT) or physical fatigue (25-min cycling). There's also a smartphone application for repeated symptom assessment during the day and a wearable for heart rate, activity level and skin temperature.

10) Although we have questions about this group, these experiments seem quite useful in figuring out muscle fatiguability (a key symptom of Ramsay ME).

If you do write about PEM, please can you make sure it clarifies that PEM is not the same as PEF, DOMS, fatiguability, exertion intolerance, though pwME can also get these.

I agree. As a long term ME patient I can say almost 100% of doctors do not really know what PEM is, even if they have heard of the concept. They still think “fatigue” not “fatiguability”, nor “neuro-endocrine-immune flare” nor anaerobic thresholds, brain-body shutdown etc.

I got a gym routine from the physio to cope with muscle fatiguability on the field a couple of years ago and I’m so angry that it works. I have to do meta exercise now

Respectfully I disagree. The rule change reverts to the status quo before last fall. Pain, weakness, limitation of motion, fatiguability,flare-ups are still considered such as in 38 CFR 4.40, 4.45 and 4.59. This is just not using an examiner’s hypothetical estimations on an unmediated state

she’s plunked into not just an adult body, but an adult body with EXTREME agility and strength with no fatiguability. So she’s got zero impulse control or inhibition that comes with aging and experiencing, even with programming to simulate it. Leading to possible DEADLY trauma induced kid tantrums.

🔬 We found that myosin behaves abnormally in relaxed muscle from human XLMTM patients and dog/mouse models, preferring a structurally disordered and highly energy-consuming biochemical state. This increase in energy usage could contribute to muscle fatiguability. (3/5)
