If we are to rescue the #NHS , we need to start with rescuing #GeneralPractice first.

Vast majority of patients contacts and #patientcare starts and ends in #TeamGP .

No solution to the troubles of the NHS can be found without supporting it.

As GPs we are very familiar with supporting our patients when demands tip them into crisis

Sadly many GPs find themselves in this position, with increasing demands, little support & failure of leadership to listen & provide what’s needed

How many more leaving will it take?

So much work done by GP practices is unseen

The media focus purely on appointments - 30 million a month & up 5 million since 2019

Imagine if appts are up 20% what is going on behind the scenes

All this with 20% £/patient since 2016 in real terms cuts

Today like every week day 1.3m people will consult in General Practice
1/30 will need emergency care at A&E
1/5 will need a referral to another NHS service for investigation or treatment in the future
GP practices have been dealing with 20% more patients than 2019

The failure to invest in GPs, with a cut in funding of 20%

Led to unemployment & under employment of GPs, practices closing & GPs leaving

The new Govt said it would bring 1000 GPs via ARRS JUST 75 have been employed

I think the equivalent in #GeneralPractice is if a woman over a certain age presents every week with a UTI, give her vaginal oestrogen!

To be honest, I’d give it to every woman on her 50th birthday!

ARRS will not be cut with the current NHS England board in place

They think it’s working despite the 31k staff adding just 40m appts for £1.4bn
Compared to 27k GPs doing 160m appts
Cost-benefit not being done at the highest levels & they actually don’t know how it works

I’m hoping that there is a big more understanding of the 34,000 GPs caring for 57million people in England

We are trying our best with limited resources

There are problems with encouraging choice for patients but also asking GPs to use advice and guidance more
If a patients insists on being referred🤷🏻‍♂️
The advice is book them in to my clinic🤷🏻‍♂️
GPs currently referring to clinics are often blocked or cancelled🤷🏻‍♂️
£20 won’t cover it🤷🏻‍♂️

The average GP practice consultation rate is 7 but this increases significantly with 1 or more health condition

10-20% of people don’t see a GP in a year depending demographics

50% of all patients are on at least one medication permanently

Data it seems to me is often used selectively. I have to be careful not to do the same. The important question is does the data fit with the experience of the staff and patients, if it doesn’t fit something is not being counted or the data is being used incorrectly.
