I use weight-adjusted LMWH VTEP. The hospital has a protocol for it too. Roughly you get a dose doubling for every 60-70kg increase in body weight

Guidelines say LMWH... But these ones are teetering on needing lysis, ECMO, etc... and I worry about bleeding from the combo of therapeutic LMWH and the fk-you doses of heparin with ECMO and/or TPA. Heparin with boluses to try to get to rapid therapeutic while presumably going for thrombectomy.

It took 7 years to recruit 51 patients for the IMPACT single-arm study on certiluzomab on top of LMWH-aspirin shows adverse outcome in 20% which is lower than non-contemporary controls.

13/ TLDR (2/2): LMWH is preferred anticoagulant for ESLD d/t safety/efficacy data. Warfarin unreliable (baseline INR variability) & DOACs have limited evidence, w/ caution advised in mod/severe liver dysfxn. Pt preferences & shared decision-making are key #medsky #pharmsky #GPsky #HMsky #emimcc
