There was a rise in shingles rates among people aged 1-29, but, once the chickenpox vaccine is introduced, the incidence of shingles in this age group actually drops (remember- these people are not eligible for shingles vaccination yet).

Age group–specific incidence increased in the earlier years of the study for the oldest age groups who were born prevaccine, and then declined in a stepwise fashion as each age group was dominated by the persons born postvaccine, starting in 1999 for 1- to 4-year-olds, 2002 for 5- to 9-year-olds, 2006 for 10- to 14-year-olds, 2009 for 15- to 19-year-olds, and 2014 for 20- to 29-year-olds (Figure 2A). In the 2 youngest age groups (1–4 and 5–9 years), incidence started to converge in 2009 with a rate of 0.5 per 1000, declining to 0.2 per 1000 in 2019. When HZ incidence was examined by each birth cohort as individuals aged, the rates in the early postvaccine birth cohort were similar to the prevaccine cohort for whom all HZ was caused by wt-VZV. On the other hand, the late postvaccine birth cohort demonstrated low HZ rates without age-associated increases (Figure 2B).

Well, after a few decades of data were gathered we had an answer about this: no dramatic spike in shingles among adults occurred (1998-2002 only have outpatient emergency department use data but the other years have both inpatient and outpatient).

The median number of total ED visits increased from 426 520 in 1998–2002 to 3 763 001 in 2015–2019; median enrollment increased from 2 273 013 to 10 066 904 during this period. From the paper: There was a total of 1 020 691 incident HZ cases among persons aged ≥30 years during 1998–2019; crude incidence increased from 4.0 to 6.3 per 1000 persons (Figure 1). HZ incidence increased with age, ranging from 2.2–4.9 per 1000 in 30- to 49-year-olds to 8.2–11.2 per 1000 in ≥70-year-olds. In the 2 oldest age groups, 60–69 and ≥70 years, rates appeared to plateau starting in 2007 and incidence in the oldest age group (≥70 years) began to decline in 2016. Rates also appeared to plateau for 40- to 49-year-olds starting in 2015 and for 50- to 59-year-olds starting in 2016. HZ incidence in ED settings, and the proportion of visits for HZ among all ED visits, showed similar patterns (Figure 1, Supplementary Figure 2).

I can't emphasize what a small fraction of the population is effected by puberty blockers and how much the folks hand-wringing about the science are claiming puberty-blockers are unsafe for gender dysphoria, but perfectly safe for their on-label uses. It is pure concern trolling.
In MarketScan®, we identified 5,174,410 eligible members 12-16 years old between October 1, 2015 and June 30, 2021, of whom 1,028 (0.02%) received a GnRH agonist (Table 2). Sixty percent of these patients were reported as female. The mean duration of GnRH agonist therapy was 251 days (median 160 days, interquartile range [IQR] 28-365 days). The most common diagnoses of interest occurring proximal to GnRH agonist use were related to gender dysphoria, present for 580 patients, of whom 329 (57%) were reported as female. Of note, gender dysphoria is not an approved use, and these drugs have not been evaluated for efficacy and safety for this indication. Other diagnoses of interest included endocrine disorder (130 patients, 13%) and short stature (110 patients, 11%). The mean duration of GnRH agonist therapy among patients with both GnRH agonist use and adiagnosis suggesting gender dysphoria was 309 days (median 241 days, IQR 77-394 days). Table 2. In TreatmentAnswers™, we identified approximately 50,000 health care practitioner office visits between 2010 and 2021 where GnRH agonist use was mentioned with patients 12-16 years old (Table 1). The most common diagnoses reported in association with these treatment discussions were disorders of puberty (35% of office visits) and congenital malformations of the uterus and cervix (24% of office visits). This data source did not report any GnRH agonist treatment mentions associated with gender dysphoria in patients 12-16 years old. Table 1. Results The sample included 5 155 282 adolescents, yielding 11 879 766 person-years of data. The rate of receiving puberty blockers among adolescents was 20.81 (95% CI, 19.04-22.59) per 100 000 adolescents assigned female at birth (AFAB) and 15.22 (95% CI, 13.73-16.71) per 100 000 adolescents assigned male at birth (AMAB). Before age 14 years, the rate of receiving puberty blockers to AFAB adolescents was slightly higher than AMAB adolescents (Figure). The rate of receiving hormones was 49.9 (95% CI, 47.14-52.65) and 25.34 (95% CI, 23.42-27.27) per 100 000 AFAB and AMAB adolescents, respectively. Hormone receipt rates increased after age 14 years, but remained low, peaking at age 17 years at 140.16 (95% CI, 128.01-152.32) per 100 000 for AFAB adolescents and 82.42 (95% CI, 73.25-91.59) per 100 000 among AMAB adolescents. Notably, no adolescents younger than 12 years received a hormone prescription. Methods This cross-sectional study used 2018 to 2022 data from the Merative MarketScan Research Database, which includes private insurance claims (eMethods in Supplement 1). This study was approved by the Harvard Pilgrim Health Care Institutional Review Board and adhered to the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines. Informed consent was not required because the data were deidentified. To capture the range of ages at which puberty begins,5 we identified adolescents who identify as TGD aged 8 to 17 years who received puberty blockers (GnRH agonists) or gender-affirming hormones, using diagnostic, drug, and procedural codes (eg, receipt of exogenous hormones, histrelin implant, diagnosis of gender identity disorder) (eTable in Supplement 1). We calculated the rate of adolescents with a TGD-related diagnosis who received this care per 100 000 total adolescents enrolled in insurance by age and sex assigned at birth. We then used the Wald method to calculate 95% CIs. We used the sex recorded at the earliest enrollment as a proxy for sex assigned at birth. Analyses were performed with SAS version 3.7 Enterprise edition (SAS Institute) in August 2024. Discussion In this cross-sectional population of insured adolescents from 2018 to 2022, receipt of puberty blockers and hormones was rare. The higher rates of puberty blocker and hormone receipt at younger ages among AFAB relative to AMAB adolescents align with their earlier onset of puberty and the current standards of care.6 This study is limited by its reliance on claims data, which may misclassify adolescents who identify as TGD and receipt of care; it also cannot determine if their use is for gender affirmation or other reasons and excludes other gender-affirming medications (eg, antiandrogens, progesterone). These data are most generalizable to those with private insurance in large group plans, likely reflecting greater access to gender-affirming care. Thus, these rates may be the highest estimates, with lower rates expected among those with less comprehensive private insurance, Medicaid recipients, and the uninsured.
