Hospice is almost always called too late.

The turning point is a family meeting where the physician confirms the prognosis clearly to everyone, at once.

Without it, families can't let go of intensive intervention, even when the patient already has.

Despite that, 75% of physicians in low-risk specialties will face a claim by age 65.
