Somatic mutations are found in only 15% of pituitary adenomas. In those causing Cushing's disease, a neurodevelopmental phosphoprotein program is reactivated and the PP2A inhibitor PPP1R17 overexpressed; in mouse pituitary cells overexpressing it, fingolimod reversed the overgrowth.

After high-risk adenomas are removed at colonoscopy, guidelines recommend surveillance colonoscopies starting at 3 years, but this screening interval is based on limited evidence. Research findings from the EPoS II trial are summarized in a new Quick Take video. https://nej.md/46XXNwO

What if adenomas are being missed not because they are invisible—but because we are looking in the wrong place?

The EYE-SIGHT trial tests whether peripheral gaze feedback can sharpen detection during colonoscopy.
EYE-SIGHT Trial: Gaze and Adenoma Detection
A multicenter RCT found real-time peripheral gaze feedback improved adenoma and polyp detection during colonoscopy without longer observation time.

Surveillance colonoscopy beginning 5 years after removal of high-risk adenomas is just as effective for detecting colorectal cancer as a shorter 3-year interval, an interim analysis of an ongoing noninferiority trial showed.

Do high-risk adenomas really need 3-year surveillance?

EPoS II asks whether 5 years could be enough—with long-term CRC outcomes ultimately deciding the answer.

After high-risk adenomas were removed, five-year colorectal cancer risk was 0.77% with first follow-up at five years and 0.82% at three years. The longer interval avoided nearly 4,200 exams. doi.org/hcjrm3
Trial suggests less frequent colonoscopies may safely follow removal of high-risk polyps
Every year, around 1.9 million people worldwide are diagnosed with bowel cancer. To prevent the disease, many countries have introduced screening programs that enable more polyps to be detected and removed before cancer develops.
