Una variante del gen SERPINE1 se vincula con una respuesta molecular distinta a mepolizumab en niños con asma, según una investigación difundida el 1 de octubre por el Hospital Infantil Lurie de Chicago. El hallazgo apunta a diferencias en la inflamación de las vías respiratorias durante infecciones virales, pero todavía no demuestra que el medicamento controle peor la enfermedad en esos pacientes. La investigación, publicada en Journal of Allergy and Clinical Immunology, reutilizó datos y muestras nasales de participantes de entre 6 y 17 años con asma propensa a exacerbaciones. Estas son episodios de empeoramiento de la enfermedad. ## La diferencia aparece en la actividad de los genes SERPINE1 codifica la proteína PAI-1. Según el hospital, los portadores de la variante producen más de esa proteína durante una infección viral, con cambios asociados a cicatrización de las vías respiratorias y una menor respuesta antiviral. Un antecedente presentado en un congreso y conservado en el repositorio de Henry Ford Health permite entender qué se comparó. Aquel análisis utilizó ARN de lavados nasales de dos grupos de investigación: MUPPITS-1, con 165 participantes, y MUPPITS-2, con 228. Esas cifras corresponden al resumen de congreso, no a una muestra confirmada de la publicación final. En ese trabajo, los niños portadores del alelo de riesgo tratados con mepolizumab mostraron mayor actividad de conjuntos de genes vinculados al epitelio y a la vía TGF-β/SMAD3 frente al placebo durante exacerbaciones. La medición describía cómo respondían los genes en las muestras respiratorias. ## El beneficio clínico todavía necesita comprobarse Rajesh Kumar, autor sénior de la investigación, advirtió en el comunicado de Lurie que el análisis no tenía suficiente potencia estadística para alcanzar una conclusión clínica. Su hipótesis es que esas diferencias podrían traducirse en una peor respuesta al tratamiento, pero esa posibilidad sigue pendiente de demostración. Para convertir el hallazgo en una herramienta de selección de medicamentos haría falta comprobar si el genotipo permite anticipar resultados clínicos, además de cambios en la actividad genética.
