I have had to go through this twice for birth control which the majority of U.S. health insurance is still federally mandated to pay for with no costsharing. There are form letters online which can help you write your appeals.
The reason insurance companies require prior auth and use robo-deny is because it makes 90% of attempts to spend their money go away, but when you fight it, they give in.

i got lucky and qualified for the thing where medicaid pays for my medicare premium. but I'm still on the hook for the costsharing stuff. surprise! $80 for going to see my primary doctor 2 weeks ago for a home health referral 🙃

(agreed about getting just enough for rent. it cut into my SNAP too! 😭)

Employee insurance open enrollment. My share of premium nearly doubles for 2026. Higher costsharing overall. New 3rd party claims management, Pinnacle... bad reviews online. Know I am lucky to have a policy at all.

also, i will say I have not had good luck talking with my company's benefits program. They refused to help me in explaining to insurance that an IUD is covered without costsharing, even though I did eventually find both the company policy and the federal law that says so.

The cuts in Medicare will be in areas (such as the funding of the residency programs) and through shifting of beneficiaries to Medicare Advantage programs, with the subsequent reduction of benefits through ⬆️ costsharing. The VBP of the next iteration of CMMI will be weaponized during that phase.

oh! I'm actually in the process of appealing my bill with my insurance -- under the ACA it should be covered with no costsharing. (The government website saying so is here: https://www.hrsa.gov/womens-guidelines)
