OK, so MLH1 is in the cytoplasm. What does it do there? Expressing patient tumor MLH1 mutations in ER+ breast cancer cells induces endocrine resistance. The cool part is that the mutations are almost always subclonal, but resist standard treatments to become the majority population over time /4

Treatment resistance is caused by a lack of cell cycle control in cells where MLH1 is restricted to the cytoplasm -- it has to be able to get to the nucleus to induce a cell cycle arrest in response to standard treatments for ER+ breast cancer patients. Primary consequence is unregulated CDK4 /5

But why is MLH1 in the cytoplasm given it's a DNA repair protein? We found that MLH1 missense mutations found in patient tumors can induce cyto localization by preventing MLH1 binding to PMS2 which is required to expose its NLS. Low levels of PMS2 have the same effect /3
