#paraprotein
The clinical vignette and findings raise a differential diagnosis of paraprotein-related glomerulopathy, fibrillary glomerulopathy, and type I cryoglobulinemia among others. The immunoperoxidase stain shown is a DNAJB9 stain that is specific and sensitive for fibrillary glomerulopathy.
What is this immunoperoxidase stain? For bonus points, what was found on electron microscopy?

Used to donate blood here in the UK, but since being diagnosed with a paraprotein 3yrs ago, my blood has been shunned 😆 I miss the free biccies and that feeling of doing something useful.

Monoclonal gammopathy, paraprotein secreted by a clonal B-lymphoproliferative or plasma cell disorder, is quite rare in children and adolescents and may be transient. Kidney injury related to…

Most monotypic fibrillary glomerulopathy is not a MGRS lesion and is being removed from the MGRS classification. An exception though is heavy chain fibrillary GN, which can be driven by a monoclonal paraprotein.

This large study revealed that nearly 10% of pts w/ MGUS have multiple paraproteins. Findings show that different paraproteins reflect relatively independent disease processes & progression risk may be similar for those with multiple & single paraproteins 👉https://bit.ly/3OAqQxy

⚡️Low HDL⚡️

It's not uncommon to see a low HDL. But the presence of newly low HDL should raise the concern for a paraprotein.

Particularly when the HDL is undetectable!

5/x clearly that did not make sense. Of note, the patient had had a ⬆️kappa 2660 mg/dL 3 mo prior. That sounded like the “hook” effect, artifactual low level of paraprotein driven by antigen excess https://pubmed.ncbi.nlm.nih.gov/29066226/
