Always look past the primary endpoint headline! KDIGO is useful, but you must understand drug physiology to read it.
MERCURI-2 nearly halved KDIGO AKI, but whether SGLT2is prevent AKI beyond oliguria remains unsettled.

That's comparing the risk of depression between those who use GLP‐1s and those who use SGLT2is. It's not an overall analysis of risk in GLP-1 users, it's that GLP-1 users have more risk than SGLT2is users. Also the cohort is entirely adults with type 2 diabetes.

Yeah, as Brian told, SGLT2is are benign drugs, however till they're proven to be effective - it would definitely constitute a risk. #NephJC

Yes, so scientifically we're justified in giving SGLT2is early. The risks of infections, and altered physiology - they're another story though! #NephJC

I am much more convinced of the benefits of SGLT2is and GLP-1s than say, Entresto, as their effects (especially of SGLT2is) are replicated and triangulated in multiple trials (rather than based on the torture of one trial).
I don't see why they would work in the acute setting though.

Sorry that it was dropped! My interpretation of DEFENDER is the same as yours, it makes me think it is safe to start SGLT2is early in the CICU in heart failure, and as diuretic adjuncts.
